D6 Accountability and ownership
Named humans for services and for release decisions.
Red flag: Nobody can say who approves production for a service.
The three statements
Each is rated from 0 (not true) to 4 (fully true). The dimension score is their average.
- Every service has a named owner.
- A named person approves production releases for each service.
- Incidents involving AI-written changes have a clear review process.
Evidence to gather
Ownership map, on-call rota, incident reviews.
Score from evidence, not impressions. Where two people score the same team, the statements they disagree on are where to look first. The assessment's compare view lists them.
First moves
If D6 is your weakest dimension, start here:
- Publish an ownership map and name a release approver for every service.
- Define an incident review for AI-written changes and run it on the next incident.
